By completing this form, you are authorizing the City of Zumbrota and the bank named in the form to initiate variable entries to your checking account. This authority will remain in effect until you notify the City of Zumbrota or the bank in writing to cancel it in such time as to afford the bank a reasonable opportunity to act on it. You can stop payment of any entry by notifying the City of Zumbrota or your bank 3 days before account is charged.

Payments will be withdrawn from your account on the 22nd of every month. If that date falls on a weekend, payment will be withdrawn on the next business day.
